The purpose of this questionnaire is to help you discover and consider issues relevant to living in community. By having a better understanding of these issues, hopefully you will be better able to decide if living in Heartwood Cohousing is right for you and you’ll be better prepared to live in community.
This questionnaire must be completed by each adult member of the household and will be used as the basis for your discussion with the Membership Team.

- What experiences have you had in any type of community settings and what did you learn from them?
- What do you think contributes to a successful community?
- What are your fears or concerns about living in Heartwood Cohousing?
- What are you most looking forward to about living in Heartwood Cohousing? What benefits do you hope to realize by living here?
- How do you deal with conflict? Give an example.
- Being willing to self-reflect and engage in personal growth is an important aspect of doing well in community. What will be an area of personal growth you anticipate might be challenging for you?
- Which of our Vision, Values, or Interpersonal Agreements resonates most with you and why do you think it is important?
- What was something in any of the communications and consensus training videos you watched that stood out to you or that you have questions about?
- Participation in community activities and work are important both for community vitality and for your integration into the community. In what ways can you imagine yourself participating? Do you have any concerns about our participation expectations included in our Community Work Agreement (contribute a minimum of 25 hours of work per quarter, serve on at least one Heartwood team)?
